W-2 Information Form for Champpions.

Original W-2’s for 2007 will be mailed no later than January 31, 2008 and will be mailed to the most recent address on your paycheck.

If that address is incorrect, the W-2 will be returned to the corporate office.  We will resend your W-2 when we receive an address correction from the link below.

If you have internet access, you can view and print your W-2 FREE after February 1, 2008.  Use the following link for instructions to register online with ADP iPay statements.  You will need your most recent paystub to register, which can be obtained at your Champps worked at location.  Prior years 2005 through 2007 W-2's are available online.

If you need to request a copy of your 2007 or a previous year W-2, please use the attached link to access the Champps request form.  There is a $10 processing fee for each W-2 requested.
